Over the past year, we’ve had the privilege of supporting several participants as they transitioned from extended hospital stays into SIL homes where they have gone on to thrive.

These experiences have offered valuable insight into the complexity of the process and what it truly takes to secure the right SIL provider while aligning multiple teams toward a successful, long-term outcome for the participant. Below are some of the key lessons we’ve learned. 

Teamwork is essential

Every team involved—within the hospital and across external services—is ultimately working for the benefit of the participant. Their collaboration is fundamental to a smooth and safe transition. 

Hospital-based teams typically include specialists, nursing staff, liaison officers, occupational therapists, mental health professionals and others depending on the participant’s circumstances. Families, public guardians (where relevant), social workers and NDIS providers planning to deliver supports post-discharge also play vital roles. 

For a transition to succeed, these groups must coordinate effectively. Open, participant-centred collaboration truly makes all the difference.

Having the right SIL provider one with experience in hospital discharge processes and a strong track record in complex transitions provides reassurance to both the hospital and the wider network that the participant will be supported by capable and well-prepared hands.

The goal is to establish the participant in a long-term home environment where their needs can be met safely and consistently.

Communication is key

While it may sound simple, a SIL provider who replies promptly to emails and queries during the transition period is demonstrating an essential skill: reliability. Effective, ongoing communication is one of the strongest indicators that a provider is organised, engaged and prepared. 

A responsive provider ensures that hospital staff, stakeholders and practitioners all receive the information they need to keep the process moving at a productive pace.

This includes providing clear and timely rosters of care, organising staff and training in advance, offering transparency regarding the home location and details about other participants (where applicable), and addressing concerns as they arise.

This level of communication empowers the hospital team to make informed decisions about the participant’s safe discharge.

Location, location, location – suitability matters

A suitable SIL home offers the right environment both inside and out.

Depending on the participant’s needs, certain factors may take priority, but typically desirable features include access to shops, public transport, community centres, parks and other amenities. 

Indoors, the home should balance safety with warmth a space that feels secure yet welcoming.

Proper equipment, an accessible layout and thoughtful design all contribute to reducing risk and demonstrating that the provider understands and prioritises safety requirements.

These elements give the hospital team confidence that the participant will transition into a well-prepared environment.

Patience and endurance 

Any major life transition requires patience, and a hospital-to-SIL move is no exception. The process must be approached carefully, with the participant’s wellbeing at the centre. 

It’s important to work with SIL providers who understand the detailed procedures hospitals must follow both in terms of compliance and safety and who are willing to move steadily, respectfully and methodically through each stage of the transition.

Hospital transitions can be complex, involving many stakeholders and moving parts.

A proactive SIL provider one who collaborates effectively, communicates clearly, prioritises safety and has a suitable home ready brings the participant much closer to a successful, sustainable transition into a place they can call home for years to come.
